HERE IS OUR PAPER APPLICATION:

Both versions are fill-able forms and can be emailed or printed and mailed.

If you are unable to download the application:
You may request an application by E-mail or Phone. The application we will send you is the same as the one you can download above. Please send your email request to admissions@circlesquare.org, or we can be reached at 212-307-0388. E-mail requests MUST include your postal (“snail mail”) address or we will not know where to send it.

If you are using a Mac:
If you are having trouble with the Acrobat Reader download, please contact Adobe’s help center. This bug has been reported to us before. REMEMBER: You may simply need to work with Mac Plug-ins for the browser you are using.
